The following statement confirms how Jayne Life Therapy Ltd (JLT), uses and protects any information that is given by anyone that uses services provided by or on behalf of JLT. It explains how JLT complies with the GDPR (General Data Protection Regulation.)


JLT is committed to safeguarding and protecting your information. If any information is asked of you, it will only be used in accordance with this privacy policy. The policy may change and be updated from time to time in line with legislation.


JLT is registered with the Regulator, the Information Commissioner’s Office (ICO) under the registration number ZB165223.


Clients:


Where necessary, in accordance with maintaining your wellbeing and adhering to safeguarding procedures, JLT may be required to process information relating to you that is sensitive in nature such as, but not limited to, health information. In certain circumstances, JLT may need to share this information with a third party such as a GP or the police if JLT considers there to be a real possibility of harm to yourself or others.


There may also be occasions where confidentiality cannot be maintained due to the level of harm posed to others for example if the protection of a child is at risk.


Further details regarding confidentiality are included in your written contract with JLT and will be discussed with you prior to working with JLT.


JLT takes measures to protect and safeguard any information it holds about you and prevent any unauthorised access as far as reasonably practicable.


How information is used:


To honour the contract between you and JLT and provide the necessary service you require.


To keep records such as client hours.


To run and maintain JLT as a business i.e. for financial records.


How information is stored:


Any information JLT holds on you is stored for a minimum of 6 years after your contract with JLT ends after which it is destroyed or, in the case of children, until the child/young person turns 21.


JLT’s computer and mobile phone are password protected to ensure the safety and protection of information.


Session notes are stored in a locked filing cabinet in a locked JLT office.


Access to information:


You can make a written request to JLT to access the information JLT holds on you at any time.


If you think any information JLT holds about you is inaccurate, you can ask for this information to be updated.
